Through a partnership with the Department of Social Development and the Town of Woodstock, our Outreach Team provides compassionate, structured, and strategic support for individuals experiencing homelessness or at risk of losing their housing.Our goal is to meet people where they are—literally and circumstantially—and connect them with the services they need.
We Are Here to Help
Our Outreach Services support communities across Western New Brunswick, including: Nackawic, Lakeland Ridges, Woodstock, Hartland, Carleton North, Southern Victoria, and the Tobique Valley.

What We Do
Our team provides respectful, persistent, and client-focused support to help
individuals achieve housing stability and exit homelessness. Services
include:
- Direct engagement with visibly
homeless individuals, hidden homeless, and those at risk.
- Completing client intake and assessments.
- Referring clients to support services and assisting
with applications.
- Accompanying clients to appointments when appropriate.
- Collecting demographic data for reporting and program
development.
- Participating in Point-in-Time (PIT) Counts.
- Maintaining contact with NB Housing Corp and related
partners to follow up on applications and client progress.
- Providing landlord/tenant mediation where needed.
- Accepting referrals from community partners including
Social Development, Horizon Health, and local faith organizations.
- Liaising with agencies and community partners to
promote and support resource use.
